PNZ Bupa Inclusive Para Sport Grant Application form

a) I believe the above information to be true and correct. I understand that this information will be used by Paralympics New Zealand to establish our organisations’ need for funding aligned with the PNZ Bupa Inclusive Para Sport Grant, and that I may be contacted by Paralympics New Zealand in order to obtain further information if required.

b) Should our PNZ Bupa Inclusive Para Sport Grant application be successful, we accept that Paralympics New Zealand may, in its sole discretion, use any information relating to the application or the applicant for the purpose of publicity to raise awareness of Paralympics New Zealand and Bupa, and the assistance that the grant provides, and the applicant consents to such use. We agree for the usage of imagery for these purposes.

c) Should our PNZ Bupa Inclusive Para Sport Grant application be successful, Paralympics New Zealand requires accountability for all funds received by the applicant. If there are changes in circumstances, an update must be sent to [email protected] as soon as possible.
