I acknowledge and agree with all of the conditions outlined in this Support Staff Application Form.

In applying to be considered for nomination and selection into a Support Staff role for the the New Zealand Paralympic Team (NZPT) at the Milano Cortina 2026 Paralympic Winter Games, I acknowledge, agree and declare that:

- If PNZ (in consultation with the relevant National Sports Organisation (NSO) certifies that this Support Staff Application is correct, I agree to complete, sign and return to PNZ at the applicable time a Support Staff Agreement which will come into effect only if and when I am selected by PNZ to the NZPT.

- In the event that my NSO nominates me to PNZ for selection as a Support Staff member to the NZPT, I acknowledge that I am not guaranteed to be selected. The decision of whether or not to select me in the NZPT is at the sole discretion of PNZ and the number of accreditations allocated to the NZPT.

- In the event I am selected I understand that I may not be accommodated in the Paralympic Winter Village. Accommodation offered may be outside of the Paralympic Winter Village at a location to be decided by PNZ.

- I have not breached, nor am I under investigation for breach of, any rules and regulations of my NSO, PNZ, the IF, the IPC or the World Anti-Doping Agency (WADA).

- I have not committed, nor am I under investigation for, any anti-doping offence as defined in the anti-doping policies of my Sports Organisation, PNZ, the IF, the IPC or Sport Integrity Commission.

- I am not currently disqualified or suspended under the rules of PNZ, the IF, the IPC or WADA.

- I can obtain the Sports Anti-Doping Rule and copies of the anti-doping rules, regulations and policies of my Sports Organisation, PNZ, Sport Integrity Commission, the IF and the IPC and I agree to be bound by them.

- Subject to the Criminal Records (Clean State) Act 2004, I have not been convicted of a criminal offence punishable by a term of imprisonment.

- I further declare that no charge in respect of any criminal offence punishable by a term of imprisonment, or anti-doping violation, is pending against me.

- I have not acted in any manner to bring myself, my Sport, my NSO or PNZ into public disrepute.

- As part of my application to be a Support Staff member of the NZPT I agree to complete the NZ Police Vetting process.

- I have not acted in any manner to bring myself, my Sport, my NSO or PNZ into public disrepute.

- I will notify PNZ of any changes to my contact details as set out above. Any failure by me to do so may be to my detriment as any announcement regarding the nomination and selection of the NZPT will be notified of these details.

- I agree to the collection of personal information about me, including size of clothing, biographical details, photos and associated imagery, the results of any health/medical examinations undertaken to asses my suitability for the Support Staff role, and the usage and storage of such information, for the purposes of consideration of nomination (if any) by my NSO and selection (if any) by PNZ.
